Q: What is "home health care"?
A: Home health care is an effective method for providing skilled and unskilled care in the privacy and security of your place of residence and allows you to recuperate. Plus, home health care gives you advantages such as:

  • Being in your own bed
  • Eating home-cooked meals
  • Unlimited visiting hours
  • Being able to relax with loved ones
  • It extends your physician's care beyond his office and the hospital, mainly when the hospital care and physician's office services have been exhausted.

Q: Do I qualify for home health care services?
A: At your request or per your physician's order, the agency nurse will conduct an initial evaluation to determine your qualification for services. Prime Care Resource, Inc. is happy to provide you with this initial evaluation free of charge. If you meet the following criteria, you will qualify for home health care:
  • If you are 65 or older, or have been entitled to disability benefits for 24 consecutive months;
  • If you are confined to your home and cannot leave without the aid of special equipment or the assistance of another person;
  • If you are under the care of a physician who has certified your need for home health care services.
  • If you came from a hospital or skilled nursing facility.

Q: Who will be coming to my home?
A: PRIME CARE RESOURCE, INC. is certified and licensed to provide you with the following services:
  • Skilled Nursing (RN)
  • Certified Nursing Assistant (CNA/HHA)
  • Physical Therapy (PT)
  • Occupational Therapy (OT)
  • Speech Therapy (ST)
  • Medical Social Worker

Q: How can you help me if I do NOT have a physician?
A: We have access to a variety of physicians of different specialties from whom you may choose or receive a referral according to your needs and your location.


Q: How can I get home health care services started?
A: All you have to do is:
  • Call our toll free number at (866) 666-6156 for a telephone screening and we will arrange an initial evaluation done by a registered nurse in your home. Again, this visit is free of charge. Or contact us via email.
  • If you qualify for home health care services, ask your physician for a referral. You have the right to request that services be provided to you by our agency.
  • If you are in the hospital, ask the Discharge Planner or the Social Worker for a referral to PRIME CARE RESOURCE, INC. and have them contact us at (847) 827-4448 or (866) 666-6156.
  • If you are being discharged from a nursing home or skilled nursing facility, seek the home health service option by using any of the above referral methods.


Q: How will I pay for your home health care services?
A: Our agency accepts a number of methods of payments. Most major medical policies from private insurance companies contain Home Health Care provisions of coverage; Verification and pre-authorization for home health care services in writing.

  • Medicare Part A & B
  • Blue Cross Blue Shield, Humana Gold, Aetna and other major private insurance companies.
  • Long Term Care Insurance
  • Private Pay
